Clinical Analysis of Nurse¢¥s Decision in Performing Dependent Nursing


Abstract
This study is a descriptive one to understand the actual condition of nurse¢¥s decision in performing doctors¢¥ orders by investigation and interview.
The object of this study were 645 doctor¢¥s orders for 114 patients on ICU, OBGY, pediatrics unit in K. hospital and nurses¢¥ decision for the performance of these orders. Data were collected from 23th. November to 28th. December 1992.
The result are summarized as follows;
In nurse¢¥s decision, the frequency of performance without any proposition of changing
orders were higher (67.2 %) than that of performance with proposition (32.9 %).
The proposition of new requirement (57.1 %) was larger that the other contents.
The factors influencing nurse¢¥s decision were classified into medical personnel factor,
pt. factors. Proposition of changing orders was presented mainly due to pt. factors. Proposition was proposed usually due to physical condition into worsening, need for prophylactic management.
According to the component of doctors¢¥ orders the highest frequency of proposition was seen at medication orders.
As above results, the conclusions are as follows;
Nurses judge whether doctors¢¥ orders are adequate or not and perform the orders. So they are making decision for the performance orders, and these decision processes form an organized decision tree.
Nurses¢¥ proposition of changing orders is presented mainly due to pt. factors that reveals the nurse is a special medical personnel working primarily for the pt..
